Issuance of an Emergency Travel Document (EU E.T.D.)

 

In emergency situations, this Consulate General may issue, to Italian citizens or citizens of EU member states not represented locally, an Emergency Travel Document, also known as an EU E.T.D. (European Union Emergency Travel Document). This document is valid only for the return trip to Italy or to the country of habitual residence abroad (only if the applicant is not a citizen of that country).

 

To obtain this document, the applicant must appear in person — by appointment to be arranged with the Passport Office (passaporti.chicago@esteri.it) — at the Consulate General, providing the following documentation:

  • In case of theft/loss of the passport completed theft/loss report form and a police report issued by the local authorities;
  • EU ETD application form;
  • Two recent passport-sized photos;
  • Copy of the passport (if available);
  • Copy of another identity document, such as an identity card or driver’s license, even in photocopy. If unavailable, the Office will carry out the necessary identity checks;
  • Travel document (airline ticket) showing arrival at the final destination.

The application for an EU ETD for a minor must be signed by both parents.

IMPORTANT: Obligation to return the document

The beneficiary of the EU ETD is required, upon arrival at their destination, to hand the document over to the border police or other police authority. If arrival is in a country other than Italy, the beneficiary must return the document to the consular office with jurisdiction over the destination area.
