Mission for family of US Military Personnel (National/long term visa)

The Consular administration has full authority to evaluate and request other documents in addition to those submitted by the applicant. The latter is hereby informed that submitting the documents specified below does not guarantee issuance of the visa.  INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED.

Please note that this visa application can only be submitted by the competent Military Department and not by individuals.

Documents check-list

  • “No fee” Passport  (plus copy of page with photo) with a validity that exceeds the intended stay of the applicant by at least three months. Please make sure your passport has two blank spaces to affix the visa. If the spouse or the children are not US citizens and connot be issued with a “No Fee” Passport, please provide their US Visa or Permanent Resident Card;
  • Visa Application Form, filled and signed by the applicant and notarized by the military  department submitting the application;
  • Recent passport-size photograph, on white background, full face and front view, stapled/glued to the application form. Scanned or photocopied photos will NOT be accepted;
  • Military orders of the sponsor assigned to Italy, including the name of the family members authorized to follow him/her;
  • Memorandum from the competent Military Department requesting a visa for accompanying family members. The request must indicate full name and family relationship with the sponsor, destination and duration of the assignment in Italy, date of departure, phone and e-mail of the officer signing the letter;
  • Original plus 1 copy of marriage certificate and child/children’s birth certificate(s) showing parents name. The original certificates will be returned to the applicant. In the event that the mother’s maiden name appears on the child/children’s birth certificate(s), also copy of the mother’s birth certificate;
  • For applicants under the age of 18, submit a notarized consent to travel form of both biological parents together with copies of ID’s;
  • Confirmed flight itinerary;
  • No visa fee is required. However if family members are non US citizens and therefore are unable to obtain a “no fee” passport, the long-term visa fee applies (under Consular Fees on home page);
  • Return envelope: required in the event that applicants cannot collect the passport with the visa in person. You must provide a pre-paid return envelope trackable (please keep a copy of the tracking number, you must be able to track it yourself). We reccomend using FedEx or UPS.
