Please notice that all the names appearing on the passport must\u00a0 appear on the application form (all data must match those on the passport). The form must be dated and signed in front of a Visa Officer;<\/li>\n<li><strong>Recent passport-size photograph<\/strong>, on white background, full face and front view, stapled\/glued to the application form. Scanned or photocopied photos will NOT be accepted;<\/li>\n<li><strong>Valid passport plus 1 copy<\/strong> of the page with photograph and expiration date. The passport or travel document must be valid at least three months after the visa expiration date. Please make sure the passport has at least two blank pages to affix the visa;<\/li>\n<li><strong>For Non-US citizens: original and copy of Permanent Residence Card<\/strong> OR other US immigration document allowing re-entry into the United States (e.g., valid long-term US visa, Advanced Parole, etc.);<\/li>\n<li><strong>Copy of Driver\u2019s License or State ID<\/strong> as proof of residence in the jurisdiction of this Consulate General. Full-time students may also use original and copy of Student ID. If you recently moved and have not updated your ID, you must submit another proof of current address (e.g., utility bills, bank statements, etc.);<\/li>\n<li><strong>Official documentation<\/strong> certifying that the applicant is an <strong>Ordained Minister<\/strong> of a church\/religious institution.<\/li>\n<li><strong>Official letters of the Religious Institutions in the US and in Italy<\/strong> sponsoring the applicant. The letters must state the exact period of stay in Italy, full financial responsibility and complete medical\/hospitalization coverage;<\/li>\n<li><strong>Copy of the Decree issued by the Italian Ministry of Interior<\/strong> by which the inviting Religious Institution has been officially recognized and authorized to operate in Italy;<\/li>\n<li><strong>Applicant\u2019s declaration<\/strong>, as well as of any accompanying adult family members, stating that he\/she will not engage in any employment\/work activity during the entire stay in Italy;<\/li>\n<li><strong>Confirmed Flight itinerary;<\/strong><\/li>\n<li><strong>Visa fee (under Consular Fees on home page):<\/strong> (non refundable) in the form of Money Order or cashier\u2019s check, payable to the Consulate General of Italy in Chicago, with applicant\u2019s name and address as the remitter;<\/li>\n<li><strong>Return envelope:<\/strong> required in the event that applicants cannot collect the passport with the visa in person. <strong>You must provide a pre-paid, self-adressed, return envelope, trackable. Mailing label sections \u201cTO\u201d and \u201cFROM\u201d must BOTH contain applicant\u2019s Name and Address (please keep a copy of the tracking number, you must be able to track it yourself).
